RESIDENTIAL TRANSITION SERVICES Coalition for Barrier Free Living offers consumers with a disability: “Residential Transition Services” Do you or someone you know have a disability and reside in a nursing facility but desire to transition back into the community? Did you know our services are FREE (for all who qualify)? The Residential Transition Services department can assist with the following: Home and Community-Based Services and other community resources Housing options or transitioning to a friend’s or relative’s home Access to grant funds to assist with moving expenses, deposits, household items, start-up groceries, and clothing Coordination of services to ensure individuals receive the necessary services to make a successful community transition, such as: Attendant Care Therapies and Rehabilitation Minor Home Modifications Medical Equipment and Supplies CBFL Residential Transition Services department serves consumers in the following 13 counties: Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Harris, Liberty, Matagorda, Montgomery, Walker, Waller, and Wharton.
